For Your First Visit

  • Arrive 15 minutes early to check in

  • Bring insurance card and photo ID

  • Bring copy of referral if you have one


Our office is not a fragrance-free facility.

Weekend massage appointments:  we do not have reception on the weekends.  The door will not be open more than 15 minutes prior to your scheduled time.

Please inform us ahead of time if you have any specific needs that we should be aware of. Thank you!


New Patients

We are accepting new chiropractic and massage therapy patients!

If you are interested in becoming a new patient, we look forward to working with you!   The very first step is to complete the general Intake Forms so we can create your patient profile.  We can offer you possible appointment times, but need to have your intake forms submitted before we can schedule your first appointment.  Don't hesitate to contact us at any time with questions.

If you are a new patient that is seeking treatment for an auto injury, you will additionally need to complete the Auto Injury Form before your first appointment:

There is no need to complete patient intake forms again unless your information has changed or if your last visit was more than a year ago.

If you are an established patient and are seeking treatment for a recent motor vehicle accident, you will need to complete the Updated Intake Form (above) and the Auto Injury Form here:
