For Your First Visit

  • Arrive 15 minutes early to check in

  • Bring insurance card and photo ID

  • Bring copy of referral if you have one


Our office is not a fragrance-free facility. Most days our office dog, Blaze, is at the clinic. He is quiet and gentle and usually keeps to himself.

Weekend massage appointments:  we do not have reception on the weekends.  The door will not be open more than 15 minutes prior to your scheduled time.

Please inform us ahead of time if you have any specific needs that we should be aware of. Thank you!


New Patients

We are accepting new chiropractic and massage therapy patients!

As a new patient to our clinic, please first contact us by clicking the contact us button below to tell us how we can help.
After you have submitted your message to us, then click the
intake forms button to complete your Intake Forms.

If we provide you with available new patient appointment times, your appointment is NOT confirmed until you have submitted your intake forms.

If you are an Existing Patient

ALL established patients need to complete our updated Missed Appointment Policy Form Here.

There is no need to complete patient intake forms again unless your information has changed or if your last visit was more than a year ago.

  • If you have not been to see us in 3 years, please complete the Intake Forms listed under the New Patients section above. 

  • If you  have not been to see us in 1-3 years, please complete the Updated Intake Form.

Are you an Auto Injury Patient?

Existing patients involved in a motor vehicle accident need to complete our auto injury intake and questionnaire form.
